2019年3月15～17日の3日間、TKPガーデンシティPREMIUM秋葉原(東京都千代田区)において、「仮説に則った臨床研究を自ら立案・主導し、日本から世界に向けてエビデンスを発信できる、がん領域における次世代のClinical Investigatorの育成」を目指した “Clinical Research Skill Advancement Workshop” の7回目となるワークショップが開催されました。
　第7回のワークショップでは、欧米の医師主導臨床研究において一般的な「未承認薬・適応外薬を用いた早期臨床試験（第I相もしくは第II相）」の実施を目指し、本ワークショップに共催いただいた製薬企業の開発パイプラインも対象薬の候補とした上で、プロトコールコンセプトシートを参加者から募りました。
　ワークショップには、米国テキサス大学MDアンダーソンがんセンターから、臨床試験の経験豊富な医薬統計学者１名と医師(臨床医、バイオロジスト)2名、日本からはがん臨床試験のリーダー6名と医薬統計学者１名が講師として招聘され、Clinical Investigatorにとって必要な知識についての講義ならびにメンターとしての役割を担っていただきました。
　本会では、3日間にわたってインタラクティブ・セッションが4回行われ、その成果として、参加者は応募時に作成・提出したプロトコールコンセプトシート（LOI）を完成度の高いものに仕上げるとともに、自身のコンセプトのプレゼンテーションススライドを作成するなかで、クリニカルリサーチデザインの完成プロセスのあり方を学びました。
　以下に、3日間のワークショップの模様を写真とコメントでご紹介いたします。

　　“The 7th Clinical Research Skill Advancement Workshop” was held at the TKP Garden City PREMIUM Akihabara, (Chiyoda-ku, Tokyo, Japan), from March 15th through March 17th, 2019 the objective of the workshop was to cultivate “the next generation of clinical investigators in the cancer research field in Japan” who can formulate and lead hypothesis-driven clinical research and disseminate evidence from Japan to the world.
　At this 7th workshop, we aimed at “early clinical trials (phase I or phase II) using unapproved agents and/or off-label drugs,” which are common in physician-initiated clinical studies in the EU and the US. We solicited the participants for protocol concept sheets (LOI) using agents including the ones in the development pipelines of pharmaceutical companies that co-hosted this workshop.
　 The workshop invited one clinical statistician, one medical oncologist and one biologist with broad experience in clinical trials from The University of Texas MD Anderson Cancer Center (U.S.A.) and six Japanese clinical trial leaders and one clinical statistician as its facilitators. These faculty members offered lectures for clinical investigators and took the role of mentors at the workshop. 2 guest speakers, one is researcher, Dr. Kudo, the other one is patient advocator (Ms. Nao Nakajima).
　 Throughout the three-day workshop, there were four interactive sessions where attendees revised the protocol concept sheets that they had submitted when applying to the workshop. They refined and created presentation slides for their protocol concept sheets. Through this experience, they learned the development process of clinical research design.
The following are highlights of the three-day workshop, including some photographs.

　本ワークショップの議長である朴成和氏のイントロダクションのあと、上野直人氏より、ワークショップの目的とその概要についてのプレゼンテーションがありました。引き続き、共催企業からパイプラインについての説明と質疑応答がありました。3日間のワークショップでは、講義とディスカッションからなる4つのワーク・セッションが行なわれました。1日目のワーク・セッションは、4つのトピックを取り上げました。最初は「Work 1: Statistical Considerations for Early Phase Trials (室谷健太氏 久留米大学)」、「Work 2: Statistical Considerations for Contemporary Clinical Trials” (J. Jack Lee氏 MDアンダーソンがんセンター)」 の2つの講演がありました。インタラクティブ・セッションでは、臨床試験の対象薬別に4つのグループに分かれ、参加者は各自のプロトコールコンセプトシートを発表し、グループの皆でディスカッションしました。最初のインタラクティブ・セッションのあと、次のワークでは「Work 3: (1) “What is the Minimum Preclinical Requirement for Submitting a Clinical Trial LOI (Chandra Bartholomeusz 氏、MDアンダーソンがんセンター)”」「 (2) “Pragmatic Approach for Conducting Translational Research in Oncology　(上野直人氏、MDアンダーソンがんセンター)”」の2つの講演がありました。

Firstly, Dr. Boku gave opening remarks and Dr. Ueno presented workshop overview.
Then, 2 sponsors (Daiichi Sankyo and Taiho Yakuhin) reviewed their pipelines and had Q&A from participants.
　Each day of the workshop included four “Work” sessions of lectures and discussions. The first day’s Work session focused on four topics: “Work 1: Statistical Considerations for Early Phase Trials from Kenta Murotani, Ph.D. Biostatistics Center, Kurume University)” and “Work 2: Statistical Considerations for Contemporary Clinical Trials from J. Jack Lee, Ph.D., Department of Biostatistics Univ. of Texas M.D. Anderson Cancer Center”.
For the interactive session, attendees were divided into four (4) groups by agents, and then presented and discussed their protocol concept sheets in their groups. After the 1st interactive session, we covered 2 topics for Work 3: (1) “What is the Minimum Preclinical Requirement for Submitting a Clinical Trial LOI” from Chandra Bartholomeusz, M.D., Ph.D. (2) “Pragmatic Approach for Conducting Translational Research in Oncology” from Naoto T. Ueno, M.D.,Ph.D.

　2日目のワーク・セッション、Work 4 では5つのテーマを取り上げました。 (1) “IIT Experiences”　(清水俊雄氏 国立がん研究センター)、 (2) “PK/PD Analysis for Drug Development in Oncology”（今村知世氏 昭和大学先端がん治療研究所）、 (3) “Immunological Approaches to Treatment of Cancer”（スペシャルゲスト工藤千恵氏 国立がん研究センター研究所）(4) “An Update on Liquid Biopsy for Cancer Research”（鶴谷純司氏 昭和大学先端がん治療研究所）、(5) “Post LOI Story” (朴成和氏 国立がん研究センター中央病院)について講義がありました。さらに、スペシャルゲストに中島順氏（乳がんサバイバー）を迎え、上野直人氏とともに臨床試験を推進するための寄付を集める活動についてのプレゼンテーションがありました。ファカルティ、製薬企業を含む参加者は、中島順氏の活動内容に強い感銘を受けました。2日目の、インタラクティブ・セッションでは、1日目と同様に対象薬別に4つのグループに分かれ、各自のプロトコールコンセプトシートの革新性・影響力・仮説・リサーチデザイン・実行可能性・プレゼンテーションスキルについて、グループの皆でディスカッションしました。最後のインタラクティブ・セッションでは、各参加者はプロトコールコンセプトシートを完成度の高いものに仕上げるとともに、自身のコンセプトのプレゼンテーションスライドを作成しました。また、“Meet the professors”のセッションでは、参加者の個人面談の機会がもたれ、それぞれの課題について話し合われました。

　The second day’s work session 4 covered 5 topics, (1) “IIT Experiences” from Toshio Shimizu, M.D., Ph.D. (National Cancer Center Hospital) (2) “PK/PD Analysis for Drug Development in Oncology” from Chiyo Imamura, Ph.D., (Showa University) (3) “Immunological Approaches to Treatment of Cancer” from special guest Chie Kudo, Ph.D., National Cancer Center Research Institute (4) “An Update on Liquid Biopsy for Cancer Research” from Junji Tsurutani, M.D., Ph.　D (Showa University) and (5) “Post LOI Story”, Narikazu Boku, M.D., Ph.D. talked about the experiences submitting their LOI to the pharmaceutical companies and the required documents for submission.
Then, we had a Patient Advocacy Session. Dr. Ueno and a guest speaker, Ms. Nao Nakajima who is a breast cancer survivor, presented their advocate and fund raising activities for promoting clinical research. This session had a high impact on all participants including sponsors and faculty members.
In the interactive sessions, as on the previous day, the attendees were divided into four groups based on agents and discussed innovation, impact, hypothesis, research design, feasibility and presentation skills of their protocol concept sheets in their groups. All attendees refined a protocol concept sheet of their own and created presentation slides of their protocol concept sheet during the evening interactive session. At a “Meet the professors” session, attendees had opportunities for one-on-one meetings and talked about each issue.

　3日目は、参加者全員によるプロトコールコンセプトシートのプレゼンテーションスライドの発表があり、発表ごとに活発なディスカッションが行われました。発表修了後、参加者の質問に答える形でパネル・ディスカッションが行われました。そして最後に修了式が行われ、ファカルティメンバーから各参加者にワークショップ修了証が授与されました。

　On the third day, all attendees presented their presentation slides of the protocol concept sheets, and each presentation was followed by a lively discussion among the participants. Following each presentation, faculty members encouraged all participants to ask the presenter questions which led to many fruitful Q & A sessions. The day ended with a completion ceremony at which faculty members awarded a certificate of completion of the workshop to each attendee.
